seeking a compassionate and driven Physician to build and manage a
dedicated panel of patients across independent and assisted living

healthcare organization. Essential Duties & Responsibilities: -
Patient Panel Development: Build and maintain strong patient
relationships by developing a dedicated panel and providing
continuity of care. - Direct Patient Care: Conduct assessments,
manage chronic conditions, and deliver preventive services as the
primary physician for residents. - Routine Rounds: Perform regular
visits at assigned facilities, addressing both ongoing and acute
health needs. - Care Coordination: Partner with facility staff,
families, and interdisciplinary teams to create personalized,
holistic care plans. - Documentation & Compliance: Ensure accurate,
timely EMR documentation while adhering to all regulatory
standards. - Patient & Family Education: Provide education on
preventive health, chronic condition management, and wellness
strategies. - Quality Improvement: Participate in initiatives that

clinical decision support Qualifications: - Medical Degree (MD or
DO) from an accredited institution - Board Certification in Family
Medicine, Internal Medicine, Geriatrics, or Hospital Medicine -
Active, Unrestricted Medical License in the state of practice - DEA
Certification and current BLS/ACLS certification - Proficiency in
EMR systems and timely documentation practices - Strong
Interpersonal and Communication Skills, with a commitment to
patient-centered care This role requires travel to and from one or
more assigned facilities. Depending on the amount of travel
required, Curana may in future determine that a valid driver’s
license, current auto insurance, and an acceptable driving record
are required. If so, you will be provided with thirty (30) days
