Medicine Physician (MD or DO) to join a dynamic team in South Bend,
Indiana . This role offers an opportunity to make a meaningful

and underserved populations in an outpatient setting. Whether
you're a seasoned provider or a recent graduate, this is your
chance to be part of a collaborative, patient-centered care model
with integrated services across medical, dental, and behavioral
health disciplines. Key Responsibilities: Provide comprehensive
outpatient primary care to a diverse patient population Collaborate
with a multidisciplinary team including physicians, nurse
practitioners, dentists, and behavioral health specialists Maintain
accurate documentation using eClinicalWorks (eCW) EMR Participate
in quality improvement initiatives and team-based care Support and
embody the mission of providing compassionate, affordable care to
all Work Schedule: Monday to Friday daytime hours (no evenings)
Four-day workweek optional No weekends required Major holidays off
Dedicated administrative time included in weekly schedule What We

accredited medical school Completion of an accredited residency in
Family Medicine or Internal Medicine Board-Certified or
Board-Eligible Eligible for medical licensure in the state of
Indiana Active DEA license (or ability to obtain prior to start)
Strong commitment to community health and serving underserved
populations Prior experience or interest in working within FQHC,
