supported About the Role: Extended Care Registered Nurse RN In this
rewarding after-hours, on-call role, you will: Triage and manage
after-hours patient needs via phone and onsite visits Conduct
hospice informational visits and nursing admission assessments
Support patients and families by collaborating on individualized
Plans of Care Work alongside an interdisciplinary team to educate,
advocate, and provide holistic, compassionate care Deliver
exceptional service while making a real impact during the
