in sub-acute rehab settings. Collaborate with experienced
physicians, therapists, and healthcare teams to improve patient
outcomes in senior living facilities. Enjoy a flexible, full- or

management to reduce readmissions. Responsibilities include
partnering with therapy teams, communicating with primary care
