What You'll Do: Provide compassionate patient care by triaging phone calls, completing patient care visits, attending death visits, and ordering supplies and equipment. Lead and supervise weekend care teams, ensuring smooth operations and continuity of care. Conduct hospice assessments , symptom management visits, and support patient admissions. Triage patient and family phone calls , providing timely clinical judgment and guidance. Support and educate families , caregivers, and patients during critical end-of-life transitions. Collaborate with interdisciplinary teams to maintain consistent, compassionate care. Mentor nursing staff , promote team cohesion, and uphold care standards during Weekend hours.
