Weekend Registered Charge Nurse- Extended Care Grand Rapids, MI

matters most. What You'll Do: - Provide compassionate patient care
by triaging phone calls, completing patient care visits, attending
death visits, and ordering supplies and equipment. - Lead and
supervise weekend care teams, ensuring smooth operations and
continuity of care. - Conduct hospice assessments, symptom
management visits, and support patient admissions. - Triage patient
and family phone calls, providing timely clinical judgment and
guidance. - Support and educate families, caregivers, and patients
during critical end-of-life transitions. - Collaborate with
interdisciplinary teams to maintain consistent, compassionate care.
- Mentor nursing staff, promote team cohesion, and uphold care
standards during Weekend hours. What You Bring: - Michigan
Registered Nurse license (required) - Minimum 2 years nursing
experience, 4 years preferred - Hospice or leadership experience is
preferred - Strong communication and decision-making abilities -
EMR proficiency - Independence and clinical confidence - BSN
