Job Responsibilities

• 
  
• Rounding and managing wound care of patients at assigned skilled nursing facilities
  
• Collaborating with wound care nursing staff to assess and treat patients
  
• Providing high-quality, patient-centered, comprehensive wound care services
  
• Timely documentation of patient care and encounters
  
• Availability for phone and secure messaging consultations with facility nursing staff
  
  
  Requirements and Qualifications
  
  • 
    
  • Valid PA/NP license in the state of practice
    
  • Valid NP/PA degree from an accredited institution
    
  • Active DEA license
    
  • General post-acute/SNF knowledge or experience preferred
    
  • Familiarity with EMR systems and charting practices
