AFTER HOURS TRIAGE POSITION SUMMARY5 pm to 8 am; 7 days on 7
days off (Tuesday - Monday rotation)Hybrid Remote Position:
Combination of phone triage (90% est) and visits (10% est)$10,000

organizations serve people in 30 counties.ENTRY REQUIREMENTS
Graduation from a recognized school of nursing and a current
Michigan RN licensure. Two years of acute care experience required.
Home care or hospice experience preferred. Registered nurses hired
by Munson Home Health or transferred from Munson Medical Center to
Munson Home Health prior to January 1, 2009, are encouraged to
obtain a BSN. Certification in the specialty area of practice is
also encouraged. Registered nurses hired by Munson Home Health or
transferred from Munson Medical Center to Munson Home Health on or
after January 1, 2009, are required to obtain a BSN within ten
years of their system hire date. Certification in the specialty
area of practice is encouraged. Proven ability to communicate
effectively in verbal and written English. Computer literacy
required. Proven health assessment, organizational, and
interpersonal skills with the ability to make independent judgments
directing patient care. Meets Home Health and Medicare standards as
evidenced by criminal background check and fingerprinting. Meets
MHH established requirements for proof of health status. Must
possess current Michigan motor vehicle license, ability to drive a
car, and an insured vehicle capable of transport to patient homes
or other offices in various weather conditions; be physically able
to lift 35 pounds; and demonstrate ability to move, position and
transfer clients utilizing good body mechanics, lifting techniques
and/or transfer assistive devices to avoid manipulating more than
35 pounds.ORGANIZATIONReports to the Director of Home Care and
Hospice, or designee.Interacts interdependently with other Home
Health staff and volunteers, physicians, support staff, other
hospital staff and members of the community.SPECIFIC DUTIES
Supports the Mission, Vision and Values of Munson Healthcare.
Embraces and supports the Performance Improvement philosophy of
Munson Healthcare. Promotes personal and patient safety. Uses
effective customer service/interpersonal skills at all times.
Provides phone triage and after hours admissions/visits for home
care and hospice patients during non-business hours. Works
cooperatively to coordinate care with colleagues to ensure that
patient and family needs are met during non-business hours.
Implements plan of care, coordinates services, and revises plan of
care as necessary while documenting in computerized medical record
in a timely manner. Performs initial and ongoing assessments of
clients' needs. Serves as liaison between physician, hospital and
agency personnel, and others in assessing appropriateness of
patient referral and ongoing patient care needs. Counsels the
client and family in meeting nursing and related needs. Attends
appropriate staff meetings and in-services/continuing education
offerings. Meets established productivity standards. Employees with
email are required to maintain proficiency in the basic functions
of the program and are also required to regularly check email and
keep calendars up to date. Complies with standard precautions,
infection control and safety procedures and guidelines. Performs
other duties and responsibilities as assigned.
