Cherry Health is a Federally Qualified Health Center (FQHC) with a mission to improve the health and wellness of individuals by providing comprehensive primary and behavioral health care while encouraging access by those who are underserved.The Community Health Worker works under the supervision and direction of the RN, LMSW and Site Manager, the Community Health Worker is responsible for improving the health and well-being of the population served, assisting high-risk families' access to available community resources and providing education related to the focus of the community program. This is a full-time, non-exempt, benefit eligible position. Typically working Monday-Friday, from 8:00am-5:00pm.Cherry Health requires all team members, including temps, contractors, interns, residents, students, etc. to receive the COVID-19 vaccinations subject to certain exclusions.     • High school diploma required
    • College coursework preferred
    • Previous experience in a health care or community setting; experience in related program and volunteer work preferred, or an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job listed above.Responsibilities:
      • Communicate information from the patient, family, physician and other members of the health care educational team to the RN, LMSW, and MD/Mid-level in order to facilitate coordination of services.
      • Provide input into the assessment of the social setting affecting the patient, make physical observations, and screen program participant per standard protocol and procedure.
      • Work collaboratively with patient, community agencies, and volunteers to schedule and provide services to family, to facilitate a supportive relationship and to evaluate effectiveness of services provided and plan for future needs.
      • May assist patient and/or family member with any available Medicaid enrollment services.
      • Identify educational needs of family, community, and volunteers and provide educational materials and basic information.
      • Provide and document services, education and other pertinent program data.
      • Mentor, empower and advocate for family to help gain independence and skills.
      • Ensure that processes and services are continuously monitored for quality, cost effectiveness, and efficiency. Engage in process and quality improvement activities. Make and implement recommendations to improve operational efficiency and to implement new services for areas of responsibilities.
      • Ensure that services are provided in accordance with the State and Federal regulations, organizational policy, and accreditation/compliance requirements.
      • Engage in continuous study, including best practices to maintain the professional competence, knowledge and skills necessary for the performance of all assigned responsibilities. Complete required continuous training and education, including department specific requirements.
      • Ensure that patients, their families, visitors, customers, and employees are treated with kindness and respect. Demonstrate teamwork and integrity in all work activities.
      • Understand and adhere to agency's compliance standards.
      • Other duties as assigned.
