RN Case Manager - Home Visits, Grand Rapids AreaJoin Our Team as an RN Case Manager for Home Visits in the Grand Rapids Area! Atrio Home Care, the partnership of Holland Home, Clark Retirement and Resthaven, is seeking a compassionate RN Case Manager to join our Grand Rapids Team. This position will involve providing direct care to clients within a 35-mile radius of Grand Rapids.ResponsibilitiesPerform initial and comprehensive assessments and deliver compassionate care to clientsCollaborate with physicians and other care team members to develop a Plan of CarePromotes client and family/caregiver independence through counseling and educationCoordinate client discharge planningIdentify barriers to desired outcomes and evaluates outcomes of carePrepare clinical documentation, progress notes in a timely manner using iPads for the EMR systemParticipates in on-call rotationLocation: Grand Rapids, covering a 35-mile radius around GRSchedule/ShiftMonday - Friday; Day shift 8-5pm, with flexibility in scheduling to help you maintain a healthy work/life balanceOn-call rotation every 6-8 weeks to cover PRN visits, on-call triage and weekend admissions, etcCompensationOur compensation model includes bonuses for high-quality client outcomes and efficient case managementPremium pay for weekend & holiday visitsQualificationsLicensed RN in Michigan; BSN preferredValid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits1-2 years of acute care or skilled nursing experience; prior certified home health experience preferredPassion for delivering high quality care to older adultsBenefitsPaid Time Off (PTO)Medical, Dental, Vision, STD, LTD, Life Insurance CoverageGenerous 403b match with access to a professional Financial Planner at no costWhy work for Atrio?Provide one-on-one quality care where you can case manage and provide ongoing visits for your own clientsMeaningful work with a supported team that makes a difference in clients' livesAutonomy within a supportive team of healthcare professionals dedicated to support youPremium pay for weekend and holiday visitsA strong faith based nonprofit organization that prioritizes quality care and a positive work environmentApply online at Home - ATRIO HOME CARE and click on the 'Join our Team' link on the home page.
